What Is Lyme Disease?
Lyme disease is a tick-borne illness caused primarily by the spirochete bacterium Borrelia burgdorferi — transmitted through the bite of infected black-legged ticks. First formally identified in Lyme, Connecticut in the 1970s, it is now the most reported vector-borne disease in the United States, with an estimated 476,000 new cases diagnosed annually according to the CDC.
The classic presentation of acute Lyme disease includes:
- The characteristic bull’s-eye rash (erythema migrans) present in approximately 70 to 80 percent of cases
- Flu-like symptoms, fever, chills, fatigue, muscle aches, and headache
- Joint pain and swelling
- Neurological symptoms in some cases, facial palsy, meningitis, and shooting pains
When caught early and treated with an appropriate course of antibiotics, many patients recover fully. The controversy and the tragedy begins when Lyme is not caught early.
Chronic Lyme Disease — The Contested Reality
Here is where science, politics, and the patient experience diverge dramatically.
The conventional medical position holds that Lyme disease is cured by a standard two-to-four-week course of antibiotics and that patients who continue to experience symptoms afterward are suffering from Post-Treatment Lyme Disease Syndrome (PTLDS), an immune-mediated condition that is not caused by ongoing infection and does not require further antibiotic treatment.
The integrative and functional medicine position, supported by a growing body of research and the lived experience of hundreds of thousands of patients, holds that Borrelia burgdorferi is capable of persisting in the body despite antibiotic treatment, particularly when treatment is delayed or inadequate, and that chronic, active infection contributes significantly to ongoing symptoms in many patients.
What is not disputed is this: a significant population of patients who have been diagnosed and treated for Lyme disease continue to experience debilitating, life-altering symptoms for months to years after their initial treatment. Whatever we call it, chronic Lyme, persistent Lyme, PTLDS, or multi-systemic infectious disease syndrome, these patients are genuinely suffering and deserve effective, compassionate care.
Why Is Lyme So Difficult to Diagnose?
The diagnostic challenges of Lyme disease are a significant contributor to the chronic illness burden associated with it. Standard Lyme testing, the two-tier ELISA and Western blot protocol, has well-documented limitations:
- False negatives are common — particularly in early infections before antibody levels have risen, and in patients with immune dysregulation who mount poor antibody responses
- Testing misses co-infections — Lyme ticks frequently carry multiple pathogens simultaneously including Bartonella, Babesia, Ehrlichia, and Anaplasma and co-infections that significantly worsen the clinical picture and require different treatment approaches
- Borrelia has multiple strains — standard tests are calibrated primarily for B. burgdorferi sensu stricto and may miss infections with other Borrelia species including B. mayonii, B. miyamotoi, and European strains
- The bull’s-eye rash is not universal — the absence of the classic rash does not rule out Lyme, and many patients, particularly those bitten in areas difficult to see, never notice the rash at all
The result is that many patients spend years receiving diagnoses of fibromyalgia, chronic fatigue syndrome, multiple sclerosis, lupus, rheumatoid arthritis, or psychiatric conditions while the underlying Lyme infection continues to drive their symptoms unaddressed.
